Ethical Foundation
BACB Code 2.15 — Interrupting or Discontinuation of Services
“It is our ethical obligation never to abandon a client. It is our moral obligation to find replacement services so that your child does not have a lull in services if services were to terminate.”
Reasons for Discharge
Goal Achievement
- Client has met treatment goals
- Client is ready to transition to less intensive services
- Skills have generalized to natural environment
Family Request
- Parents have the right to withdraw services at any time
- Document the request and reason if provided
- Offer transition support
Insurance/Authorization Changes
- Authorization not renewed
- Insurance coverage changes
- Work with family to explore options
Clinical Recommendation
- Services no longer clinically indicated
- Client would benefit from different service model
- Refer to appropriate resources
Discharge Process
1. Transition Planning
- Begin planning before discharge date
- Identify ongoing needs
- Connect family with resources
- Train caregivers on maintenance strategies
2. Documentation
- Complete final progress report
- Summarize goals achieved and current status
- Document recommendations for continued support
- Ensure all session notes are finalized
3. Family Communication
- Discuss discharge timeline with family
- Review progress and achievements
- Provide recommendations for maintaining gains
- Offer information about future services if needed
4. Handoff (if transferring)
- Coordinate with new provider if applicable
- Share relevant documentation (with consent)
- Provide transition summary
- Offer consultation period if appropriate
Never Abandon a Client
If Bee-Have Cannot Continue Services
- Provide adequate notice to family
- Assist in finding replacement services
- Ensure no lull in client’s therapy
- Complete transition documentation
Staff Leaving a Case
- Provide advance notice
- Participate in transition meetings
- Brief incoming staff thoroughly
- Complete all documentation before leaving
Discharge Documentation
Required Elements
- Discharge summary
- Final progress on all goals
- Recommendations for continued care
- Caregiver training provided
- Resources and referrals given
Return of Materials
- Any Bee-Have materials at client’s home
- Data collection tools
- Visual supports (copies can remain with family)
Insurance Considerations
6-Month Reporting Cycle
- Insurance requires reassessment every 6 months
- Goal: move client toward least restrictive environment
- Document progress toward discharge throughout treatment
Authorization Tracking
- Monitor remaining authorized hours
- Plan discharge timing with authorization periods
- Avoid abrupt endings due to authorization expiration
